Quick Referral Form

Referral Service for Dentists

Thanks for your referrals.

Already the practice receives a significant number of referrals for complex or difficult problems, enjoying the challenge they present. We welcome such referrals and invite you to send (or continue to send) them to us. When you do refer to us, you may find it helpful to complete our quick referral form.

Please include any recent radiographs (which we shall scan into our computer before returning the originals to you), indicate to which dentist you are referring (if you have a preference), make it clear whether you are asking for a second opinion, treatment-planning recommendations, or whether you are asking us to carry out specific treatment or to take on comprehensive care of the patient.

What are the costs to the patient?

We are a private practice and the first consultation appointment with us costs £60.

If a patient has complex problems further time may be required at a later visit to complete examination and treatment planning. If further time is required and agreed with the patient at this first visit, then the cost of subsequent visits will be given to them in a written estimate.

When treatment options are being discussed with the patient, costs of the various options will be discussed.

Once a treatment plan (or a stage of treatment) is agreed, then a written treatment plan / estimate is given to the patient.
